Social Worker | Geriatric Care

Job Description :

• Create personalised care plans tailored to the needs of the resident
• Implement care plans and regularly record residents' progress
• Design and organise programs and activities to address residents' care requirements
• Prepare and submit review reports and monthly interview records every 6 month
• Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents

Job Requirement :

• Min Diploma/Degree in Social Work/Psychology/Counselling
• At least 1 year of relevant healthcare industry experience
• Good interpersonal and problem-solving skills
• Proficient in English and able to converse in dialects or other languages
• Office Hours: 5 days work week
