Medical Social Worker / Case Manager (West)

Roles and Responsibilities

• Provide counseling service to patients and care givers
• Provide psychosocial and financial assessment, coordinated case management and interventions to enable the patient/client to function optimally in the home environment and community at large
• Assist patient to understand the diagnosis and implication on ADL functioning
• Walk through with patients and care givers and provide emotional support and motivation for them
• Manage patients expectation and set realistic and achievable goals
• Arrange for interim care or make referrals for long term step-down facilities when necessary
• Participate in ward rounds and case conferences to represent patients' social, psychological and economic circumstances.
• Responsible for Volunteer Management
• Plan and oversee the volunteer programs, not limited to recruitment, screening, coordination, training and retention of volunteers
• Undertake projects and any other duties as assigned by the Management

Requirements

• Master/Bachelor/Post Graduate Diploma in Social Work
• Registered Social Workers with Singapore Association of Social Work (SASW) - will be an advantage
• Compassion and care for patients
• Good team player with positive attitude, caring and compassionate individual
• Good interpersonal skills, able to communicate people at all levels such as patient’s families, government agencies and other professionals
