Physiotherapist (Healthcare/6.8k/5 days)

Responsibilities:

• Conduct accurate and holistic physiotherapy specific assessment for clients during admission, periodic care plan reviews and discharge
• Select appropriate and specific outcome measures to enable evaluation of therapy progress and outcomes in a timely manner to guide clinical decision making
• Formulate person centered and SMART goals in collaboration with clients and relevant stakeholders
• Design plans and conduct physiotherapy interventions for clients in a person centered manner, that is also evidence based
• Document assessment results, therapy plans, interventions, goals and outcomes measures according to guidelines
• Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and relevant stakeholders
• Ensure proper use of equipment including technological-assisted rehabilitation equipment
• Formulate discharge and continuity of care plans based on clients’ performance in consultation with relevant stakeholders
• Facilitate community integration through collaboration with other community partners as required
• Empower clients and caregivers with methods to enhance health, mobility and functions through self-management strategies  outcomes.

Requirements:

• Diploma or Degree in Physiotherapy and must be AHPC registered
• Minimum 2 years of neuro-rehabilitation experience
• Good team player and able to work independently
